A fat alderman with an elaborate wig strolls with a woman, presumably his wife, as in the Biblical creation of Eve from Adam's rib. At the time their clothing was seen as being in extravagant continental style, the term macaroni being applied to fops and dandies and to those who emulated continental fashions.
